Memory Care Coordinator
Sunday to Thursday 8:30 am to 5:00 pm
Salary / On Call (shared rotation)

Responsibilities:
- Implement and administer personalized Wellness Care Plans for Residents
- Timely response to all emergency calls
- Ensure that a personal visit occurs following an incident, accident, injury or return from hospital
- Respond and report Resident/family requests/concerns to Wellness Director and/or Wellness Coordinator
- Respond and report Resident’s change of condition and/or needs
- Identify, assess, report and document the clinical status of Residents
- Observe and report any significant changes in Resident behavior and health to the Wellness Director and/or Wellness Coordinator, physician, family, and Resident
- Administer medications and treatments as ordered by physician
- Accurately documents on the administration records as directed in Manor Village policies and procedures
- Assist with move-ins, discharges and transfers of residents
- Review physicians’ orders to ensure accuracy of medication and treatment administration
- Order necessary medications and treatment supplies for resident
- Maintain medication and treatment carts and medication room in clean orderly fashion
- Maintain record of medication room refrigerator temperature.
- Accountable for narcotic count and records
- Assist in electronic medication administration and electronic health records and other audits as directed by the Wellness Director and/or Wellness Coordinator
- Maintain shift updates in communication log and participates in shift exchange meeting daily
- Serve as mentor to new wellness employees (caregivers)
- Assume leadership responsibilities as directed by the Wellness Director and/or Wellness Coordinator in the implementation of Resident wellness programs (Yardi)
Qualifications:
- Current Caregiver Certification and/or LPN
- Arizona Fingerprint Clearance Card
- Current First Aid and CPR certifications
- Must be 18 years of age or older
- Experience working with dementia or Alzheimer’s residents preferred
- Background clearances as required by government regulations
- Complete a health screening and provide negative TB test results (must be within the last 6 months or within 7 days of employment)
- Proficiency in verbal and written English
- Ability to transfer, assist and lift up to 50 lbs.
- Strong work ethic, dependable and reliable
- Open availability to work flexible days
