The Maples Assisted Living Facility, LLC -
A Personal Care Class will be offered in our facility. When you pass class you will be a certified PCA and will be offered employment at The Maples Assisted Living Facility. The class will teach you to do or recognize the following :
Encourages and reinforces need for safe environment related to equipment, mental status changes, fall precautions, first aide. Hallways are free of equipment, supplies and debris.
Involves resident / family in care decisions : Allows residents to maintain control; involves and encourages self determination and independence.
Explains all care procedures, explaining to resident, prior to performance
Recognizes and reports any abuse or neglect potential
Knowledgeable and follows service plan as planned by Care team
Provides feedback and suggestion when changes occur in service plan
Assist with eating, care of teeth, grooming, bathing, restroom, dressing, walking, as required within Service Plan
Assist therapist, as needed with rehabilitation services as requested
Reports any change in the patients mental and physical condition to supervisor
Will NOT perform the following activities : Foley Catheter Irrigations, Sterile Dressing Changes, Enemas, or application of heat in any form.
Follows Chain of Command to ensure proper communication
Ensures 12 hours of Inservice education completed annually
Meets current documentation standards and policies
Follows through as needed on information given by shift report, resident or family regarding suggestions, changes or concerns
Treats residents and their families with respect and dignity
Demonstrates competence in technical and manual skills according to scope of practice for PCA
Provides reminders for meals and medications
Functions as a resident advocate by protecting and ensuring residents rights
Observes and ensures the environment is well maintained and assists in providing a safe environment for residents, staff and visitors
Knowledgeable of work order and communicates necessary issues to environmental services
Communicates with Interdisciplinary team to ensure residents needs are met
Consistently follows the policies and procedures of the home
Assists with the self-administration of medications and follows medication rights and reduces the potential for medication error (TRAINED AS MED TECH ONLY)
Reports any incidents or accidents to supervisor
Maintains First Aid Certification
Professional Requirements :
Adheres to dress code, wears name badge, appearance is professional, neat and clean. No long artificial nails, no large or long dangling jewelry, no large dangling earrings.
Completes annual education requirements.
Maintains regulatory requirements and active NYS certification.
Maintains resident confidentiality at all times.
Reports to work on time and as scheduled, completes work within designated time.
Wears identification while on duty, uses computerized punch time system correctly.
Completes education / in-services in a timely fashion. Monthly and annually and as requested.
Attends annual review and department inservices, as scheduled.
Attends staff meetings such as shift huddles / reports.
Represents the organization in a positive and professional manner.
Complies with all organizational policies regarding ethical business practices.
Communicates the mission, ethics and goals of the facility.
Makes eye contact, smiles and greets others by name
Make an effort to get to know team members, residents and their families.
Learn about and deliver on the needs of the residents
