Job Description: We are seeking an empathetic Social Worker to support residents and their families, addressing emotional, social, and practical needs while ensuring their well-being. Monday - Friday, Day Shift

Key Responsibilities

• Conduct assessments and develop individualized care plans.
• Advocate for residents’ rights and preferences.
• Provide counseling and emotional support.
• Coordinate discharge planning and community resources.
• Maintain accurate documentation and regulatory compliance.
• Facilitate group activities or workshops.
• Collaborate with healthcare professionals and families.
  
  

Qualifications

• Bachelor’s Degree in Social Work or other human service field (psychology, sociology, health & human services, etc.)
• Experience in healthcare or long-term care preferred.
• Strong interpersonal and communication skills.
• Knowledge of state and federal regulations.
• Proficiency in care planning software.
