JOB SUMMARY : Responsible for the implementation and monitoring of patients in the Care Connections program. Works in concert with therapy department providing communication on patient performance, compliance, and tolerance. Maintains accurate records of patient participation and / or refusals.
ESSENTIAL DUTIES & RESPONSIBILITIES :
Maintains a current list of patients on the Care Connections program.
Is aware of and performs duties and patient care according to facility policy and procedure.
Performs care connection program activities according to the comprehensive care plan for those patients assigned to program.
Carries out pre-determined care connection program activities for patients including but not limited to :
functional mobility consisting of ambulation and standing
application of splints, cones, AFO’s, and other orthotics as per therapy recommendations
ROM exercises as per therapy recommendations
Explains to the patient that care connection activities must be performed.
Verbally communicates daily with therapy representative regarding each patient who is on a care connection program, noting any variance from the patient’s care plan.
Reports any change in condition or response to unit coordinator / nursing supervisor, and / or Therapy Director.
Completes any other duties as assigned by Therapy Director.

Exhibits and fosters a spirit of teamwork, positive attitude and conflict resolution among co-workers.
Performs duties with measured timeliness and accuracy.
