Assistant Director of Nursing

3PM-11:30 PM 80 Hours Bi-Weekly Monday-Friday

Salaried position

Assistant Director of Nursing position is responsible to supervise day-to-day nursing functions, while administering quality nursing care to residents, assessing residents’ health problems and needs, developing and implementing nursing care plans in conjunction with the Interdisciplinary Team, and evaluating and managing medical resident care. Manages all floor nursing employees during their assigned shift. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems through the grievance process.

Evaluates nursing activities to ensure resident care, staff relations, and efficiency of service.

Observes nursing care and visits residents to ensure that nursing care is carried out as directed, and treatment administered in accordance with Physician's instructions.

Prepares and assists residents for eating and drinking during meals, transports residents to dining room, assures food is accessible, feeds residents requiring assistance, prompts residents requiring cueing, supervises self-feeding residents, cleans residents after meals with face washing, hand washing, clothing, trays, wheelchairs, tables, passes trays, etc.,

Makes sure all residents are offered nourishment and hydration at meals and during the shift.

Directs preparation and maintenance of residents' clinical records.

Reviews physician orders to ensure accuracy of MAR, TAR and other documentation.

Reviews transfer sheet, medications, diet and MD orders; obtain other orders as necessary (e.g. therapy, podiatry consult, etc.)

Completes head to toe evaluation of new admissions/readmissions for completion of the following UDAs: Admission, Braden, Fall, Wander and Pain evaluation and other evaluations, such as SBAR, etc.

Assists in the transfers and lifts of residents as needed; walks ambulatory residents as needed.

Inspects rooms and resident care areas for cleanliness and comfort.

Accompanies Physician on rounds and keeps informed of special orders concerning residents.

Completes appropriate sections of UDA for incident/accident reports and assesses resident for injuries via completion of body audit and obtains/records vital signs, as indicated, such as temperature, pulse, blood pressure, and other vital signs.

Answers call lights, personal alarms and residents’ needs and responds to the resident request, for assistance with ADL’s, such as toileting, repositioning/boosting, hydration, transfers and socialization, etc.

Participates in orientation and training of personnel.

Orders/directs ordering of drugs, solutions and equipment and maintains records on narcotics.

Investigates and resolves concerns/complaints and completes concern form along with corrective action/response; refers unusual problems to superior.

Obtains report from off going shift; presents report to on coming shift. Reports all changes in conditions etc. to the designee.

Identifies safety hazards; initiates corrective action; initiates immediate action in emergency situation involving resident conditions

Follows residents rights program and abuse prevention protocol.

Participates in educational activity and coordinates activity with Director of Staff Development/DON.

Attends staff meetings, participates in employee disciplinary measures, assumes responsibility for compliance with all appropriate rules, regulations, etc.

Completes reports as assigned, developed care plans, administers and documents medication.

Ensures pertinent medical information needed to care for resident is located in resident plan of care, to include person-centered interventions.

Accurately and objectively document pertinent resident information in the clinical record following the facilities policies and procedures regarding documentation

Performs IV insertions, administration of IV medications/ fluids for those with PICC or midlines, or other RN specific clinical duties, ie portacath care, removal of PICC lines/ midlines.

Assist med nurses with procedures as needed - catheters, bladder scans

Follows facility admissions policy and facility discharge and transfer policy

Participates in resident care plan meetings as requested

Counts all narcotics in accordance with established facility policy

Supervises and supports resident care, prepares and administers tube feeding, medications etc.

Notifies the physician of changes in condition, labs, x‑rays, etc.

Performs resident care as per other duties as assigned and as indicated in job duties

Initiates and participates in Cardiopulmonary Resuscitation

Maintain resident confidentiality at all times

Sufficient experience in order to perform daily tasks with an understanding of the duties involved. Prior experience as a Staff Nurse is required. A minimum of one year as a Charge Nurse or supervisory experience in an assisted living facility is required.

· LICENSES/ REGISTRATIONS

  • Associate’s degree from an accredited school of nursing is necessary. Completion of Bachelor’s degree is preferred.

· Maintain a current, valid license as a Registered Nurse in the Commonwealth of Pennsylvania

  • Stay updated on advancements in nursing practices, physiology, anatomy, and healthcare regulations.

Experience

  • Proven experience in nursing administration or a similar leadership role within a healthcare setting.
  • Strong knowledge of physiology and anatomy as it relates to patient care.
  • Familiarity with resident rights and advocacy within assisted living or home care environments.
  • Experience managing acute care situations effectively while ensuring quality outcomes.
  • Excellent communication skills with the ability to lead a diverse team.
  • Understanding of regulatory compliance requirements related to nursing practice.
