Job Description:

• Contribute to patient assessments and develop individualized care plans in collaboration with registered nurses.
• Administer medications, monitor patient responses, and provide holistic health education to patients and their families.
• Identify potential safety hazards and take corrective action to ensure resident well-being.

Required Skills and Qualifications:

• Current LPN licensure in the state of Ohio.
• Basic Life Support (BLS) certification within 30 days of hire date.
• High school diploma or equivalent.
