Job Description:
- Assess patients' physical and emotional needs, and develop individualized care plans in collaboration with registered nurses.
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with daily living activities.
- Identify potential safety hazards and take corrective action to ensure resident well-being.
Required Skills and Qualifications:
- Current LPN licensure in the state of Ohio.
- Basic Life Support (BLS) certification within 30 days of hire date.
- High school diploma or equivalent.
