RN Case Manager

Job Summary
The Registered Nurse Case Manager plays a crucial role in coordinating patient care and ensuring that individuals receive comprehensive and effective medical services. This position involves working closely with patients, families, and healthcare teams to develop and implement personalized care plans, monitor patient progress, and facilitate communication among all parties involved. The ideal candidate will possess strong clinical skills, a deep understanding of case management principles, and a commitment to providing high-quality patient care.

Duties

• Assess patients' health status by obtaining vital signs and conducting thorough evaluations.
• Develop individualized care plans based on patient needs, preferences, and clinical guidelines.
• Coordinate with interdisciplinary teams to ensure seamless transitions of care across various settings.
• Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
• Provide education and support to patients and families regarding treatment options and health management.
• Ensure compliance with HIPAA regulations to protect patient confidentiality.
• Advocate for patients within the healthcare system, including navigating managed care processes.
• Collaborate with behavioral health specialists when addressing mental health concerns.
• Utilize knowledge from critical care settings, including experience in a Level II Trauma Center or ICU, to inform case management decisions.
• Maintain accurate documentation of patient interactions and care plans in accordance with facility policies.

Skills

• Proficient in assessing vital signs and understanding critical health indicators.
• Strong knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
• Experience in behavioral health settings is preferred for holistic patient management.
• Background in critical care nursing or experience working in a Level II Trauma Center is advantageous.
• Familiarity with managed care principles and case management strategies.
• Competence in medical-surgical nursing practices is essential for comprehensive patient assessments.
• Excellent communication skills for effective collaboration with healthcare teams and patient education.
• Strong organizational skills to manage multiple cases efficiently while ensuring high-quality care.

Job Type: Full-time

Pay: $72,800.00 - $83,200.00 per year
