Our organization is seeking qualified (LPN) - Hospice Nurse to work with our geriatric population in their own homes or within Independent Living Facilities. Responsibilities include comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. The nurse will work as a liaison between physicians, families, and the administrative nursing team to ensure a continuum of care.
Additionally, you will have the opportunity for salary increase upon completion of your CHPN (Certified Hospice & Palliative Nurse) at no charge to the employee. We support and encourage professional development within our team.
The ideal candidate will have Hospice experience and the following qualifications:
- Willing to travel to multiple locations
- Valid LPN ( Nurse) License in good standing with the State of Michigan
- Ability to work independently as well as within a team environment
- Exceptional interpersonal & communication skills
- Outstanding organizational and self-management skills
- Proficient computer skills
- Auto Insurance and reliable transportation
- 2 year of nursing experience required
