What are the responsibilities and job description for the Licensed Practical Nurse (LPN) - Private Duty position at The Medical Team, Inc?
Essential Functions:
- Provides services in accordance with treatment plans and plans of care.
- Performs or assists in performance of procedures in accordance with physician's orders and within the realm of practice for LPN.
- Communicates identified patient needs to the RN and/ or Clinical Services Manager consistently and in a timely manner.
- Notifies RN and/ or Clinical Services Manager immediately of any significant changes in a patient's condition.
- Participates in monthly case conference.
- Provides information regarding patients' rights and responsibilities and treatment plans to patients and/ or family members.
- Responds to, and communicates, patient questions, concerns, and/ or complaints in a timely and professional manner.
- Implements current nursing practice.
- Assesses patient condition/ needs in every home health care visit.
- Administers prescribed medications and treatments as permitted by State or local regulations.
- Transcribes, implements, and evaluates outcome of physician orders.
- Maintains accurate, complete, and timely clinical notes and incorporates into clinical record at least weekly.
- Implements appropriate infection control and safety measures and reports all hazards and accidents in a timely/ accurate manner.
- Maintains confidentiality of patient information.
- Promotes and maintains an Agency environment compliant with federal, state, and local regulatory health and safety standards.
- Uses equipment and supplies safely and efficiently.
- Communicates information regarding patient equipment and supply needs to appropriate individuals in a consistent/ timely manner.
- Demonstrates accurate and efficient utilization of equipment and supplies.
- Communicates information regarding malfunctioning equipment and/ or inadequate supplies to appropriate individuals.
- Communicates information regarding safe and efficient use of equipment and supplies to patients and their families.
- Provides information about reimbursement of needed equipment and supplies to patients and families.
- Actively participates in professional growth and development.
- Participates in regular educational programs.
- Demonstrates current knowledge of the principles of nursing practice.
- Provides input to the development and revision of Agency clinical and administrative policies and procedures.
- Participates in the development and presentation of educational programs as appropriate.
- Maintains current professional license.
- Attends Agency in-services.
- Participates in the Performance Improvement Program.
- Oversees clients and services in our non-clinical settings, e.g. Independent living buildings and private communities.
- Ensures that client's needs are being met thru various non-clinical county programs.
- Participates in Blood Pressure Clinics, Flu immunization Clinics, lab draw clinics, and Health Fairs as directed.
- Redirects evidence of skilled care needs to Clinical Division as indicated for appropriate follow up.
- Fulfills additional performance responsibilities as assigned and as necessary.
