Our organization is seeking qualified NP to work with our geriatric population in their own homes. Responsibilities include comprehensive assessments, individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. The nurse will work as a liaison between physicians, families and the administrative nursing team to ensure a continuum of care.
Provides care and treatment to patients under the supervision of the attending physician(s) in accordance with established guidelines.
*The ideal candidate will have Hospice experience and the following qualifications:
- Willing to travel to multiple locations throughout the Metro Detroit Area
- Valid NP License in good standing with the State of Michigan
- CPR/BLS Certification
- Ability to work independently as well as within a team environment
- Exceptional interpersonal & communication skills
- Outstanding organizational and self-management skills
- Proficient computer skills
- Auto Insurance and reliable transportation
- 2 year experience in hospice setting
