Resident Care Manager/LPN

The Medical Team, a full-service healthcare organization, is seeking an experienced and compassionate Resident Care Manager/LPN for our Independent Living facility. The Medical Team provides personal care for seniors in independent and assisted living facilities (such as; showering assistance, toileting assistance, med passing, escorting to and from the dining area, and help dressing and grooming).

Essential Functions:

• Oversee daily activities, manage staff and activities directly related to patient's care
• Perform or assist in performance of procedures in accordance with physician's orders
• Communicates identified patient needs and any significant changes in a patient's condition consistently and in a timely manner.
• Provides information regarding patients' rights and responsibilities and treatment plans to patients and/ or family members.
• Responds to, and communicates, patient questions, concerns, and/ or complaints in a timely and professional manner.
• Regularly monitor medication cart and perform weekly med cart audits.
• Participate in arranging and conducting orientation / in-services for nursing assistants/ patient care attendants.
• Fulfill additional performance responsibilities as assigned and as necessary.

Qualifications:

• Minimum 5 years Nursing experience
• Minimum of 3 years experience as a Nurse Supervisor
• LPN License in good standing with the state of Michigan
