RN Clinical Manager

The Medical Team currently has a position opening for a Full-Time RN Clinical Manager - this is NOT a travel/contract position - This position is Full-Time M-F with excellent benefits. We offer a positive work environment and mentorship program.

Responsibilities may include any and/or all of the following:

Oversight and Supervision of Personnel, Patient/Staff Assignments, and Coordination of Care. Responsible for assuring communication and integrating orders among physicians/practitioners, and clinicians. Review new patient referrals, coordinate plan of care based on level of care needs, approval of frequencies, receive and transcribe verbal orders, mange staff schedules. Ensuring the coordination of services and interventions are provided to clients timely and in accordance with agency policy, procedure, federal and state regulations.

The ideal candidate will have several years of home health or hospice experience, excellent customer service and organizational skills, and experience in WellSky (software) and Microsoft Outlook.

Qualifications include:

• RN license - current and unencumbered
• CPR/BLS Certification
• Must demonstrate competence to render care independently and in accordance with standards of nursing practice.
• Ability to work independently as well as within a team environment.
• Minimum of 1-2 years experience in home health. Hospice experience acceptable.
