Summary :

Provides comprehensive education to patients with diabetes and their families. Performs assessments, planning, delivery, evaluation of care, and education related to each patient's age-specific needs. Works with the Diabetes Education Team on program / curriculum quality improvement. Adheres to approved / recognized standards of care for nursing, recognized nursing techniques, procedures, and administrative policies. Maintains the accurate / timely documentation of care that reflects the patient's condition, treatment, and response to care. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications :

Required : Bachelor of Science in Nursing (BSN) Degree from an accredited school of professional nursing. Possess a current Registered Nurse (RN) license through the State of Ohio Board of Nursing. Meets one of the following :

  • Certified as a Certified Diabetic Educator (CDCES) from the National Certification Board for Diabetes Educators (NCBDE).
  • 2 years of experience in diabetes care and education. NOTE : A CDCES certification must be obtained within 2 years of the hire date. Additionally, the CDCES certification must be maintained throughout employment with The MetroHealth System. Extensive knowledge of diabetes, including the disease's process, associated treatments issues, insulin / medications used in treatment, potential complications, and psychosocial issues impacting patients. Extensive knowledge of American Diabetes Association (ADA) Standards of Medical Care, Practice Recommendations, and requirements for maintaining status as a Recognized Diabetes Self-Management Education program. Able to work independently and as a member of a multidisciplinary team. Able to manage multiple and / or simultaneous responsibilities and prioritize one's work schedule. Able to maintain confidentiality for all medical, financial, and legal information. Able to maintain positive working relationships with other employees and medical staff. Excellent verbal, written, and interpersonal communication skills, and excellent problem-solving skills. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred : Master of Science in Nursing (MSN) Degree from an accredited school of professional nursing. Currently a CDE. Knowledge of basic EPIC functionality. Physical Requirements : May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate face-to-face, via phone, email, and other communications.
