Summary:
Job Summary: Manages the clinical and day-to-day operations of the care management teams. Collaborates with leadership for care coordination across the continuum of care. Performs the care coordination duties of collaborating with the interdisciplinary medical teams in the provision of patient care, for performing a clinical review of selected patients to determine service needs, and for coordinating the care of selected patients within the organization. Integrates and coordinates access and utilization management, proactive patient management, care facilitation, and treatment planning functions. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
