- Enhance quality of life by promoting empowerment, safety, and respect.
- Ensure health and well-being by attending health care meetings and managing medical appointments and follow-ups.
- Oversee medication management, including orders, records, and audits.
- Provide crisis intervention and emergency services.
- Implement strategies to achieve ISP goals.
- Act promptly in the event of abuse/neglect complaints, including follow-up actions.
- Manage and update electronic health records.
- Prepare and review documentation and reports accurately and punctually.
- Support ISP process with assessments, goal setting, and progress summaries.
- Oversees all aspects of medication treatment and administration, including preparing Health Care Provider Orders, Medication Administration Records, Current Medication Lists, Weekly MAP Checklists, Monthly MAP Checklists, Medication Occurrence Reports (HCSIS) and ensures corrective action required by a MAP Audit occurs.
- Minimum of an associate degree or equivalent, or a high school diploma/GED with 3 years of relevant experience.
- Bachelor’s degree preferred.
- At least two years of experience in human services required.
- Supervisory experience is a plus.
- Commitment to individual empowerment, human rights, and community integration is essential.
- Must be familiar with internet usage, word processing, electronic file management, and email systems.
