Social Worker
The Monarch at Brooklyn Rehab and Nursing Center is seeking a talented Social Worker to join our team
Social Worker Responsibilities Include:
- Assumes authority, responsibility, and accountability for the delivery of medical related social services in the facility. Collaborates and coordinates in an efficient and economic sound process with the interdisciplinary team.
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
- Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Social Worker Qualifications/Requirements:
- BSW or MSW preferred, or Bachelors degree in related field
- At least one year experience in Social Work field required
- Long term care and/or sub-acute setting preferred
- Ability to work flexible schedule
- PCC experience preferred
- Knowledge of MDS 3.0 preferred

Salary: Up to $35.00 an hour (Based on Experience)
