What are the responsibilities and job description for the Clinical Residential Coordinator position at The Moore Center?
Our amazing Clinical team is growing and looking for a Clinical Residential Coordinator to ensure quality, safe and individualized services for adults with developmental disabilities in a Clinically Enhanced Family Care setting and in Clinical Group Homes. Will also oversee staff and providers as they support program participants.
You will:
· Train incoming and ongoing staff and providers on client special needs, behavioral and residential progress report writing, incident report writing
· Review with provider and provide trainings for paperwork compliance of related He-M State Regulations, as well as internal policies of The Moore Center
· Review client evaluations in conjunction with individual and team to implement recommendations, set goals and identify objectives
· Institute services that are based on the individualized budget, residential proposal and Annual Service Agreement
· Oversee care provided to the client by subcontracted provider
· Monitor provider residences and assigned group homes for compliance with fire safety regulations.
· Prepare certified homes for State inspections; coordinate any plan of correction
· Provide on-call coverage as assigned
· Provide crisis intervention and problem-solving regarding residential services
· Participate with team to interview and vet potential providers and staff
· Participate in all aspects of client moves, i.e. identify provider, set up placements, complete accompanying paperwork, coordinate client move, and monitor placement
· Provide emergency on call support for providers when urgent issues arise
Requirements
Requirements include:
· Bachelor’s degree in related field preferred
· A minimum of two years of direct or related experience required
· Proficiency in Microsoft Office Suite and database systems
