Job Summary: The Peer provides guidance and support to individuals transitioning back into the community after incarceration or institutionalization for mental or physical health issues. Using their lived experience, the Peer assists participants in securing stable housing, accessing employment opportunities, obtaining food security, and navigating available community resources. This role focuses on fostering empowerment, reducing recidivism, promoting recovery, and supporting successful reintegration into society.
Key Responsibilities:
Peer Support:
- Provide one-on-one and group support to individuals recently released from incarceration or discharged from mental health or medical institutions.
- Share lived experiences to foster hope and demonstrate pathways to successful re-entry and recovery.
- Offer emotional support and guidance to participants facing housing instability, employment barriers, food insecurity, and health-related challenges.
- Assist participants in setting realistic, achievable goals for personal, professional, and wellness growth.
Resource Navigation:
- Help participants access re-entry programs, housing assistance, employment resources, healthcare services, and public benefits.
- Assist with the development of personalized re-entry and recovery action plans tailored to each participant’s needs.
- Collaborate with service providers, case managers, medical professionals, and legal advocates to ensure comprehensive support.
Community Engagement:
- Encourage participants to build supportive peer relationships and engage in pro-social activities.
- Advocate for individuals re-entering the community by reducing stigma and fostering inclusion.
- Connect participants with mentorship opportunities, support groups, and recovery programs.
Documentation and Reporting:
- Maintain accurate records of participant progress, interactions, and referrals.
- Participate in incident reporting and program evaluations as required.
Qualifications:
Education and Experience:
- High school diploma or equivalent required.
- Lived experience with incarceration, mental health institutionalization, or long-term medical institutionalization, along with successful community reintegration, preferred.
- Peer Certification or willingness to obtain certification within six (6) months of hire.
Required Skills/Abilities:
- Strong interpersonal and communication skills with the ability to connect empathetically with participants.
- Ability to share personal experiences in a supportive and empowering manner.
- Familiarity with re-entry services, mental health and recovery resources, community organizations, and employment assistance programs.
- Commitment to maintaining confidentiality and respecting participant privacy.
Physical Requirements:
- Ability to work in various environments, including office settings, community locations, and outdoor areas.
Salary : $19 - $23
