Position Overview
The main function of this position is to organize, plan, supervise, and evaluate programs intended to maximize the optimum functional level for each resident. This includes instructing, developing, supervising and evaluating staff in carrying out such plans.
Essential Functions:
- Assess newly admitted residents to determine their rehabilitative potential and plan programs to meet their restorative needs and rehabilitation goals.
- Collaborate with staff to establish the restorative plan of care.
- Serve as a resource for nursing staff.
- Develop and maintain mobility and ADL programs in conjunction with staff.
- Assist with staff education related to the rehab/restorative needs of residents.
- Monitor the progress of rehab/restorative programs.
- Work closely with therapists to assume rehab/restorative responsibilities as an adjunct to skilled therapy or when formal skilled therapy concludes.
- Ensure continuity of residents' rehab/restorative regimens on the nursing units.
- Participate in residents’ care conferences and ensure restorative and safety care plan interventions are implemented.
- Monitor and ensure staff documentation of restorative programs and ADLs is appropriately completed in the computer program for each resident.
- Oversee the walk and dine ambulation program to ensure all ambulation needs of residents are met and review for appropriate documentation.
- Participate in the QA committee with individual responsibilities, including chairing QA projects related to ADL and incontinence.
- Ensure nursing standards of care and policies and procedures relative to residents’ rehab/restorative needs are followed.
- Contribute as a member of the leadership group in identifying and solving problems and formulating and implementing department objectives.
- Evaluate and write performance reviews for restorative CNA staff.
- Complete all assigned computer education modules in a timely fashion.
- Take responsibility for personal professional growth.
- Learn and use standard software functions and basic computer operations to enhance efficiency in resident/patient service, assessment, and clinical documentation. Use computers and software as required by the work environment to enhance communication, performance, and knowledge.
- Adhere to Presbyterian Living Corporate Compliance policies, including those related to confidentiality and security of information, and concern for the well-being of residents and patients. Demonstrate behavior and complete work in a manner consistent with these policies.
Skills/Qualifications
- Active, current Illinois license to practice professional nursing. BSN or equivalent preferred.
- CPR Certification Required - Accredited by American Heart Association.
- Active Restorative/Rehabilitation certification.
- Minimum of three years’ experience as a staff nurse with good assessment skills and geriatric patient experience.
- Familiar with Point Click Care EMR.
- Genuine interest in working with older adults.
- Knowledge of aging changes and common health problems of geriatric patients.
- Good communication skills.
