This is a great opportunity to provide empowering human services in our community in our Home and Community Based Services (HCBS) & Community Oriented Recovery & Empowerment (CORE) program – Per Diem.

  • $27.81/hr.
  • up to 19 -hours work week
  • M – F, Hours are based on CORE members need and Per Diem Providers Schedule

JOB DESCRIPTION: The role of the HCBS/CORE Service Provider is to assist individuals in improving their functional abilities to the greatest degree possible in settings where they live, work, learn, and socialize, including rehabilitation counseling, skill building, and psychoeducational interventions. The HCBS/CORE Service Provider is responsible for working with individuals and their identified support system, offering instruction, emotional support, and skill building necessary to facilitate engagement and active participation of the family in the individual’s recovery process.

The HCBS/CORE Service Provider will be responsible for providing the services below based on the participant’s needs:

Habilitation: These services assist participants with developing skills necessary for community living and, if applicable, to continue the process of recovery from a Substance Use Disorder. Services include things such as: instruction in accessing transportation, shopping and performing other necessary activities of community and civic life including self-advocacy, locating housing, working with landlords and roommates and budgeting. Services are designed to enable the participant to integrate full into the community and endure recovery, health, welfare, safety and maximum independence of the participant.

Family Support and Training (FST): Service Provider offers training and support necessary to facilitate engagement and active participation of the family in the treatment planning process and with the ongoing instruction and reinforcement of skills learned throughout the recovery process. This service is provided only at the request of the individual. A person-centered or person-directed, recovery oriented, trauma-informed approach to partnering with families and other supporters to provide emotional and information support, and to enhance their skills so that they can support the recovery of a family member with a substance use disorder/mental illness. The individual, his or her treatment team and family are all primary members of the recovery team.

Psychosocial Rehabilitation (PSR): This service may include the following components: Rehabilitation counseling including recovery-oriented activities and interventions that support and restore social and interpersonal skills necessary to increase or sustain community tenure, enhance interpersonal skills, establish support networks, increase community awareness, develop coping strategies and effective functioning in the individual’s social environment such as home, work, and school.

HCBS/CORE Service Provider may also provide assistance to participant with developing conversation skills and a positive sense of self; coaching on interpersonal skills and communication; training on social etiquette; relapse prevention skills; identify trauma triggers; develop anger management skills; engender civic duty and volunteerism o Community.

SPECIFIC RESPONSIBILITIES:

  • Engage individuals in shared decision making around service and recovery planning.
  • Develop an Individualized Service Plan collaboratively with clients and their identified support system.
  • Assessment of progress toward goals and updating service plans on a regular basis.
  • Communicate with other providers to ensure integrated, collaborative services and access to necessary supports/resources.
  • Assist clients in developing or improving personal coping strategies to prevent crises and promote recovery.
  • Develop, strengthen, and support the individual’s independent community living skills and community participation through self-advocacy and system navigation; promote access to necessary rehabilitative, medical, social, academic, and other services and supports.
  • Provide instruction in accessing and using community resources such as transportation, translation, and communication assistance.
  • Provide instruction and skill building to increase the individual’s capacity to independently manage their own financial resources, including public benefits and entitlements, work incentives, and earned income.
  • Assist individuals in identifying personal, environmental, and behavioral health barriers that may impede the development of skills necessary for functioning in work or school, with peers, and with friends and family.
  • Assist individual in developing/improving skills to support the individual to self-recognize emotional triggers and to self-manage behaviors related to the behavioral health disorder.
  • Connect individual/family with mediation and conflict resolution services.
  • Assist individual/family in development of coping and problem-solving skills that draw upon family’s strengths.
  • Maintain a professional matter at all times, adhering to The Neighborhood Center, Inc.’s policies and procedures and in keeping with the Agency’s Mission Statement.
  • Perform other duties as requested by Program Director or Agency Administration.

REQUIRED QUALIFICATIONS:

  • Staff must be at least eighteen (18) years of age.
  • Must possess a valid New York State Driver’s License and have transportation.
  • Must possess a High School Diploma/Equivalency Diploma and have a minimum of one (1) year of experience working with individuals with behavioral health diagnoses.

Expected hours: up to 19 hours per week hours are based on CORE Members needs and Per Diem providers schedule.
