Basic Work Schedule is: 8:30pm – 4:30 p.m.; Monday thru’ Friday with occasional nights and weekends, INCLUDING ROTATION OF ON-CALL.


JOB DESCRIPTION: The ICM Program Director reports directly to the Director of Community Psychiatric Rehabilitation services. The Program Director is responsible for leadership and administrative functions and for ensuring that services are delivered in a manner consistent with New York State Child Welfare Law and best practices. The Program Director develops policies and program procedures, in conjunction with The Neighborhood Center, Inc. and Oneida County Children and Family Services that provide a foundation to optimize the effectiveness and efficiency of the program, while ensuring compliance with regulatory guidelines. Responsibilities include supervision of staff, ensuring compliance with New York State Child Welfare Law, Oneida County Department of Children and Family Services Preventive/Protective Services’ policies and procedures, and agency policies/procedures. This position is one of independence and advanced casework skills and requires an ability to work with highly complex situations and multiple partners. Program Director serves as a role model for Case Managers in emphasizing the team approach and excellence in meeting and securing the needs of individuals and families.

SPECIFIC RESPONSIBILITIES INCLUDE:

  • Under the supervision of the Director of Community Psychiatric Rehabilitation Services, the Intensive Case Management Program Director, is accountable for the day-to-day program operations and supervision of staff, including completion of employee appraisals.
  • Ensure case management team conduct home visits and develop individualized service plans in collaboration with clients/family.
  • Ensure Home Visits are made to conduct an intake, assessment of needs, service planning, service plan implementation including information, referral and advocacy, service coordination, monitoring and follow-up, reassessment, case conferencing, crisis intervention, short term respite, empowerment, identification of natural supports, and transitioning out of the program with supports in place.
  • Plan and coordinate outreach, supervise individual and family assessments, assess training needs and provide education, home visiting, and service coordination performed by Case Managers. Accept emergency case assignments, as needed.
  • Establish relationships with program partners including probation officers, Oneida County Court personnel and individuals within the Office of Child and Family Services/Department of Social Services.
  • Provide leadership, training, supervision, evaluation and professional development opportunities for staff in accordance with agency policies and procedures.
  • Ensure systems are in place to provide effective outcome monitoring and continuous quality improvement in all program functions.
  • Maintain data in accordance with performance standards/outcome measures to demonstrate effectiveness of program design, implementation and sustainability as well as entering into web-based data systems.
  • Ensure programs and services operate to capacity including quality and quantity of services provided and individuals served in accordance with contract.
  • Ensure program compliance with New York State Office of Children and Family Services regulations in addition to all other local, state and federal governing laws and regulations.
  • Ensure utilization of best practices and research evidenced-based practices and ensure Case Managers are providing a high-fidelity wraparound approach to service delivery.
  • Prepare required reports and other written material regarding program implementation and activities.
  • Complete monthly, quarterly, yearly and all other required reports accurately and in a timely manner.
  • As a mandated reporter, ensure all incidents are reported in a timely manner, including those reported by staff. Responsible for ensuring tracking of incidents as well as assisting with investigations as requested.
  • Cooperate with agency/community partners with investigations conducted by outside entities such as Child Protective Services.
  • Attend court hearing(s) with clients/family to provide support/advocacy, and case activities as required by the court.
  • Attend various community/county meetings on behalf of the agency.
  • Interpret to staff: agency objectives, policies, personnel policies, and evaluation procedures. Ensure that orientation materials are up-to-date, distributed to staff in a timely manner, and interpreted clearly.
  • Participates in the hiring, orientation, and regular and consistent training of program staff. Directly responsible for the completion of the Supervisor’s performance evaluation and monitoring of established goals.
  • Ensure appropriate supervision by modeling and positively interpreting agency objectives, policies, and directives which include, but are not limited to: conducting periodic evaluations for staff directly supervised by this position, and ensuring all staff fulfill training requirements of agency, funding, licensing, and regulatory bodies.
  • Maintain self in a professional manner at all times and adheres to the policies and procedures of the Agency.
  • Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time and prepared to start working.
  • Employees are expected to be available to work occasional evening and weekend events as assigned, based on the needs of the program.
  • Employee must be available to participate in a rotating on-call schedules as required by the program. This includes responding to urgent matters outside of regular business hours, including evenings, weekends, and holidays.

REQUIRED QUALIFICATIONS:

  • A clean and valid NYS Driver’s License.

AND

  • Knowledge of the specific geographic area and the population to be served.

AND

  • Bachelor’s Degree in Social Work, Psychology, Human Services, or related field and two years related experience, with one-year supervisory experience in similar related position.
