Basic Work Schedule is: 8:30 – 4:30; Monday thru’ Friday 35 hours work week! With occasional nights and weekends INCLUDING ROTATION OF ON-CALL.
JOB DESCRIPTION: An Intensive Case Manager (ICM) is responsible for assisting high-risk children and youth with serious mental illness who reside in the community and are in need of psychiatric rehabilitation. The role of the ICM is to maintain and enhance the functioning level of this population to establish healthy and constructive living patterns. As such, the ICM is expected to work autonomously to address the needs of individuals with serious mental illness by providing advocacy, monitoring, linking, and support services. As a member of a treatment team, the ICM assists high-risk children and youth in establishing goals through the development of an individualized service plan. The ICM works primarily in the field providing services in the client’s home, school and in the community.
SPECIFIC RESPONSIBILITIES INCLUDE:
- Serves as an advocate, facilitator, monitor, supporter and primary resource and care coordinator for assigned clients.
- Works with the client, families and natural supports to maintain and enhance the client’s quality of life and prevent psychiatric hospitalizations and out of home placements by reducing the stresses in community life.
- Implements the principles of psychiatric rehabilitation by encouraging the empowerment of each individual.
- Conducts individual strength-based assessments and develops an individualized service plan for clients.
- Encourages participation in appropriate programming services and provides assistance as needed.
- Assists children and youth in recognizing how services may alleviate problems and help them achieve their objectives.
- Assists children and youth in exercising improved judgment skills through supportive monitoring.
- Supports and encourages clients as they attempt to adjust to new services.
- Provides care coordination and links clients with appropriate community resources and natural supports.
- Reports crisis situations and utilizes Crisis Services when set-backs indicate the need for assessment and/or stabilization
- Adheres to and maintains case record documentation within the guidelines and procedures as established by the Office of Mental Health (OMH) and Medicaid for ICM Program.
- Maintains self in professional manner at all times, adhering to Neighborhood Center policy, procedures and in keeping with the Agency’s mission statement.
- Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time and prepared to start working.
- Performs other duties as requested by Agency Administration.
REQUIRED QUALIFICATIONS:
- Knowledge of the mental health delivery system and community resources.
- Proven strong organizational, assessment and communication skills.
- Experience in securing cooperation of and working effectively with individuals and agencies in the community.
- A Bachelor’s Degree in Social Work, Nursing, Psychology or a closely related Human Service field AND four (4) years experience in providing direct services to mentally disabled clients or experience in linking mentally disabled clients to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services)
OR
- A Master’s Degree in Social Work, Nursing or Psychology or a closely related Human Service field and two (2) years of experience as per the above.
