Your Role as the MDS Coordinator RN:

Americare Senior Living is seeking a dedicated MDS Coordinator RN to coordinate MDS assessments per federal guidelines. In this role, you will ensure residents receive quality care and a home-like environment while supporting their families. You will also help maintain facility standards and assist the Director of Nursing with inspections, care planning, and staff support.

What You’ll Do:

• Coordinate and maintain all required resident documentation, including Nursing Assessments, Care Plans, and Notes, ensuring compliance with ASI, state, and federal regulations.
• Assist in keeping facility standards up to state and federal requirements and help with inspections.
• Evaluate medication systems for effectiveness and compliance with state regulations.
• Communicate significant changes in resident conditions to the administrator and ensure proper reporting.
• Assist with resident admissions, transfers, and decisions on care placement.
• Conduct nursing staff meetings, assess equipment needs, and ensure sufficient supplies.
• Complete and update assessments, care plans, and MDS sections as required, while participating in Medicare and care plan meetings.

Physical Demands:

This position requires standing, sitting, walking, lifting up to 25 pounds and using equipment for resident care. Safety procedures, including two-man lifts and the use of mechanical lifts, are essential for ensuring the safety of both staff and residents.

Qualifications:

• Graduate of an accredited school of nursing (RN).
• Current Registered Nurse license.
• Experience with MDS 3.0 and skilled nursing required.
• Familiarity with Medicare, state, and federal regulations.
• Preferred: 1 years of clinical experience in skilled nursing facilities and 2 years in long-term care.
