Job Paths Community Support Professional – Level 1

Supervision Received: Community Support Professionals work under the supervision of the Team Leader and/or Director of Job Paths

Supervision Exercised: None

Essential Programmatic Job Responsibilities:

1. Assist people being supported to obtain valued social roles within their community (domain of community) and support them in fulfilling those roles on a regular basis.
2. Develop and implement strategies based on SRV's teachings related to image and competency enhancement with the goal of assisting the person in obtaining and/or increasing the Good Things of Life.
3. Support people in projecting a positive personal appearance by assisting with personal care needs when necessary and assuring people’s attire is appropriate for the role they are fulfilling.
4. Plan with individuals, family members and significant others to develop goals that maximize their strengths, increase control over their lives, and help to enhance their opportunities to achieve socially valued roles within the role domains of relationships, work, education, recreation, community and culture.
5. Develop opportunities and expectations for people being supported to be fully engaged in their community and by using their interests, finding role domains where those interests and gifts are welcome, and support that person to participate in a meaningful way.
6. Showcase the person's interests, talents, gifts, and skills in typical and ordinary ways that community members respect, value, and find interesting. Create opportunities for the person to be welcomed by valued people (domain of relationships)
7. Support people in being regulars at local establishments and help them to contribute fully and meaningfully.
8. Support people to obtain and maintain volunteer and/or paid positions.
9. Support people in at least 1 valued social role per week on a consistent basis.
10. Coordinate and assure the safe and efficient transportation of people being supported between destinations.

Non-Programmatic Job Responsibilities:

1. Promote The Nemasket Group’s mission and vision as a Team Member and a Mentor
2. Serve as initial liaison between work, home and funding agencies with effective verbal and written communication.
3. Attend and participate in team meetings and meet regularly with Team Leader.
4. Participate in professional growth and development/training activities as required.
5. Comply with The Nemasket Group’s work schedule and attendance policy.
6. Meet dress code requirements for office and assigned businesses.
7. Work evenings, weekends and holidays when necessary.
8. Accurately articulate The Nemasket Group’s role in supporting people
9. Demonstrate effective verbal and written communication skills
10. Demonstrate strong time management, organization skills and basic problem-solving skills
11. Maintain all documentation including, but not limited to: daily progress notes, monthly summaries, work & community& and safety assessments, incident reports and physical files per program requirement.
12. Assure safe keeping of people’s funds by assuring The Nemasket Group’s practices are adhered to.
13. Adhere to agency use vehicle policy.

Other Responsibilities:

  • Have a safe, reliable, registered and insured vehicle available for business use.
  • Complete performance measurements and job standards within established time lines.
  • Perform other duties as assigned by the team and support personnel, which may not be included in this job description.
  • Must complete Tier 1 training requirements within the introductory period.
  • Attend and participate in at least 2 agency wide staff meetings annually.
  • Attend and participate in meetings as required by supervisor.
  • Must complete 4 day Social Role Valorization Workshop within the first 3 years

Minimum Qualification Standards:

  • High School Diploma or equivalent is required.
  • College credits or Associate’s Degree is preferred.
  • Experience preferred.
  • Must be knowledgeable in computer use and willing to learn and utilize the existing and planned computer networks.
