The Social Services Department in Manhattan Division of The New Jewish Home is seeking for a full time Social Worker to join their team!
Job Responsibilities:
- Completing MDS and biopsychosocial assessments.
- Facilitate care plan meetings & discharge planning.
- Implement Advanced Directives and End of Life Planning.
- Completing all required documentation in the medical record ( EMR)
- Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills
