The Adult Day Care Center in the Upper West side of Manhattan is seeking is seeking a part time ( 24 hours a week) Social Worker to join our team! The work hours are : 4 days, 6 hours per day Tuesday, Wednesday and Friday, 11:00-5:00, Monday, 10:00-4:00 and 1 Saturday per month.
Job Responsibilities:
- Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments
- Coordinate and facilitate care planning and completion of all documentation
- Facilitate team and family meetings, education on older adults rights and well being needs of the vulnerable elderly population..This involve extensive work with family members and friends as well.
