We offer long-term (nursing home) care in Manhattan and Westchester, which is person-directed and caters to the preferences of our residents. For community-based older adults, we offer adult day health care and home care services to help them remain in the community with the care and support they need. Both our Manhattan and Westchester facilities are accredited by the Joint Commission, and last year, our Manhattan facility earned the highest possible assessment by U.S. News & World Report for its rehabilitation program. Our Manhattan skilled nursing facility is the only one in NYC to staff full-time Mount Sinai physicians and nurse practitioners.
The Adult Day Care Center located at Waters Place, Bronx is currently seeking a full time Registered Nurse to join our team! The work schedule is Monday to Friday, 8:30 am to 4:30 pm
Job Summary: Responsible for providing and coordinating multidisciplinary health services for older adults in the Day Care Center . Works with the physician and other team members in implementation of plan of care.
Duties and Responsibilities:
- Provides and coordinates cost-efficient patient care that is and reimbursable under the patients’ insurance plan and approved by a physician.
- Admits patients in the program as needed and in a timely manner.
- Uses diagnosis coding skills to accurately rate the acuity of each
- Evaluates patient diagnosis and works with Director, Patient Services and physician to assign appropriate primary diagnosis.
- Participates in bi-weekly and PRN meetings to report adherence to frequency and duration of services as per physicians’ order. Reports patients’ progress to team.
- Communicates frequently with PT/OT to ensure adherence to therapy at the frequency and duration ordered by the physician.
- Acts as a coordinator of the multidisciplinary team providing care to patients.
- Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family.
- Documents content and process of all services provided.
- Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs.
- Appraises care provided, revises goals and interventions to patients’ care as needed.
- Educates the older adults and family regarding the disease process, self-care techniques and prevention strategies.
- Initiates and maintains written and verbal communications to assure optimal patient care.
- Performs other relevant duties as needed.

Actual salaries depend on a variety of factors, including experience, education, and organizational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

Requirements:
- License and current registration to practice as a Registered Professional Nurse in New York State.
- Graduate of an accredited school of nursing, BSN preferred.
- A minimum of one year experience in acute and subacute care and or minimum of one year experience in a Adult Care Center preferred.
- Bilingual ( Spanish /English) and UAS certified preferred.
Salary : $45 - $47
