Together, our interrelated programs provide a whole-person, whole-family, and whole-life approach that unlocks solutions for a lifetime.Responsible for the coordination, advocacy and follow-up of individual�s medical care also responsible for the organization and maintenance of all medical documents and medical appointments. Will also provide direct care support, supervision and assistance to individuals in Residential Program with daily living and community integration. Communicates with outside service providers and advocate for quality treatment. Schedule: Mon-Fri 7a-3p (Tuesday 8a-4) Salary: $17.66 per hour -$18.96 per hourResponsibilities
DUTIES: day-to-day responsibilities include but are not limited to: • Coordinate, transport and attend all medical care appointments for individuals residing in assigned residence(s) in a safe and timely manner. • Communicate and advocate with medical care providers, nurses, clinical, management and program staff in regards to residents medical treatment plans. • Update all medical books/documentation as necessary and communicate all changes to appropriate persons as required.• Administer medications and document books/logs accurately and in a neat and timely manner.
