The Special Care Network Enrollment Specialist work under general supervision to assist in the implementation and management of the 115 New York Health Equity Waiver Regional Social Care Networks for NYF

Work under general supervision to assist in the implementation and management of the 115 New York Health Equity Waiver Regional Social Care Networks for NYF
Responsibilities:
- Coordinates with management across NYF to ensure 100% compliance of required training is completed and documented for screening and navigation staff.
- Serves as the system administrator supporting and managing required system used for the Special Care Network.
- Identifies, trains and ensures consistent used of required AHC screening tool by screening and navigation staff.
- Sets up tracking systems, timelines and coordinates submission of all required SCN documents as per requirements.
- Works with IT to insure AHC screener is programmed correctly in Evolv and workflows between screening and navigation are correct. Serves as the point of contact to identify issues and works to quickly resolve them
- Works with finance to ensure accurate documentation for all SCN activities to support reimbursement
- Support the evaluation of the Social Care Network activities.
Salary range: $56,000 to $63,000

Qualifications
Requirements:
- Bachelor’s degree
- requires strong communication, data analytics, and performance management skills.
- 2 years of experience in a coordinator role managing multiple deadlines across departments
- Demonstrated strong commitment to safety
