Position Summary
We are seeking an experienced and dedicated Client Care Coordinator to join our growing team. This full-time leadership role plays a vital part in ensuring exceptional care for our clients by overseeing care coordination, scheduling management, and caregiver support.

Client Care Coordinator Responsibilities
Scheduling & Care Coordination
- Create and maintain caregiver schedules in WellSky, ensuring proper coverage for all clients.
- Communicate promptly with caregivers and clients regarding scheduling updates or changes.
- Resolve scheduling conflicts and handle last-minute adjustments to maintain seamless care delivery.
- Monitor caregiver availability and proactively plan for future staffing needs.
- Ensure schedules align with client care plans and preferences.
Caregiver Support & Team Coordination
- Assist with onboarding new caregivers, including verifying credentials and updating schedules.
- Review caregiver time cards for accuracy and resolve discrepancies.
- Provide ongoing support and communication to caregivers, ensuring they have the resources needed for client care.
- Track caregiver performance and provide feedback.
- Support compliance efforts by monitoring caregiver certifications, training renewals, and other requirements.
Client Relations & Service Quality
- Serve as a primary point of contact for clients and families, addressing inquiries, concerns, and service adjustments.
- Coordinate initial client assessments and ensure accurate documentation of needs and preferences.
- Develop and update client care plans in collaboration with the care team.
- Conduct regular check-ins with clients to ensure satisfaction and quality of care.
- Facilitate communication between clients, caregivers, and healthcare providers.
Emergency Care Coverage & Hands-On Support
- Step in to provide caregiving services when needed, including personal care, ADL support, and companionship.
- Ensure proper handoff and documentation when resuming normal schedules.
Requirements
Experience: Minimum 2 years of caregiving experience (required). Previous scheduling, supervisory, and/or administrative experience highly preferred.
Caregiver Certifications: Caregiver Certification, CNA License, or Certificate of Completion from a Formal Training Program. Relevant experience in acute care settings is highly preferred.
Other Certifications: Current First Aid and CPR certifications required.
Background & Health Screening: Must pass a criminal background check, drug screening, DMV record check, and possess a negative TB test prior to employment. A fingerprint clearance card is also highly preferred.
References: Must provide at least two positive professional references from previous employers or supervisors that will be verified.
Compensation and Benefits
Salaried Position: $45,000 - $55,000 per year, based on experience and qualifications.
