Community Health Worker - Columbia Gorge Pride Alliance (CGPA) Program
Worksite Location: The Dalles
Work Schedule: Full-Time; 40 hours per week; Monday - Friday, some evenings and weekends
Wage Range: $22.91 - $25.16 per hour
Bilingual Differential Pay: $0.50 per hour worked
Reports to: Health Promotion Services Program Manager

Who are we looking for?
The Community Health Worker (CHW) assists program participants in accessing services to improve or promote health and wellness. Assisting participants may include navigating program pathways, documenting progress, and empowering program participants through supportive relationships.
What you will do…
- Track policy that affects LGBTQIA2S community, following local, state and federal legislation.
- Support LGBTQIA2S community with peer and crisis support.
- Assist in planning and running educational workshops, program activities, and outreach events.
- Organize volunteers for CGPA events.
- Connect community members to gender affirming items.
- Build strong relationships with families / households by offering care that fits their culture.
- Work with community members / families / households to help them manage their own care and make decisions together.
- Find out what services are needed, what’s missing, and what’s being duplicated, and work with other organizations to fix any issues.
- Speak up for community members / families / households to help them use services they need.
- Make sure any service gaps are noted and communicated clearly.
- Use and improve local resources that are already available.
- Build good relationships with other community health workers and partners.
- Create connections with health care, dental, and social service providers.
- Attend all required training and meetings for community health workers.
- Give feedback to help improve programs.
- Complete enrollments, assessments, and follow up with clients, making sure they get the resources they need.
- Other duties as assigned by supervisor.
Education, Experience, and Requirements…
Required:
- High School Diploma or equivalent
- Community Health Worker certification or ability to obtain one within 90 days of
employment
- Connection to and experience working with the LGBTQIA2S community
- Valid driver’s license with auto liability insurance
- Successful completion of a comprehensive background check
- Experience working with Microsoft 365, and zoom or willingness to learn
- Experience working with vulnerable populations
Preferred:
- Bilingual in Spanish and English
- Experience using social media platforms, or willingness to learn
