The Night Ministry is a Chicago-based nonprofit that provides human connection, housing, and health care to individuals struggling with housing stability or poverty. Our work is centered around two core service areas: Outreach & Healthcare and Youth Housing. Through our Outreach & Health programs, we meet people where they are, on CTA platforms, in encampments, at libraries, and across city neighborhoods; offering consistent, compassionate care and vital health services.
Our Youth Housing programs provide shelter and supportive services to young adults, helping them build stability and work toward their goals in safe, affirming environments. What You'll Do
As the Case Manager Supervisor, you'll lead and support our CTA Outreach Case Managers while directly providing case management services through our Health Outreach Bus and street outreach programs. You'll play a vital role in mentoring staff, coordinating services, and ensuring that the individuals we serve receive consistent, high-quality care and referrals.
Your work will include:

Supervising the CTA case management team, including hiring, training, coaching, scheduling, and conducting performance evaluations.

Providing direct case management services and follow-up to clients encountered through the Health Outreach Bus and other outreach programs.

Maintaining accurate and timely records in systems like Electronic Health Records (EHR) and Efforts to Outcomes (ETO), and ensuring case files are compliant with agency and funder standards.

Building relationships with referral partners, including those in housing, mental health, addiction services, and spiritual care.

Supporting on-the-street staff during onboarding and as needed during CTA coverage.

Collaborating with agency leadership, attending staff meetings, and representing The Night Ministry at public-facing and inter-agency events.

Driving the outreach van as needed and contributing to outreach efforts across the city.
What We're Looking For
We're seeking an experienced, detail-oriented leader with a passion for outreach, a trauma-informed mindset, and a strong ability to support both clients and team members.
Qualifications:

Bachelor's degree in social services or a related field.

2-5 years of experience in outreach, health services, or case management.

Experience supervising or mentoring staff or interns.

Ability to work in all weather conditions and outdoor settings for extended periods.

Strong organizational, written, and verbal communication skills.

Valid driver's license, clean driving record, and access to a vehicle.

Commitment to harm reduction and competency working with LGBTQ , trauma-affected, and street-involved populations.
