We've successfully provided these lifesaving services for 45 plus years, everyday providing a lifeline to our most vulnerable neighbors. Position Summary:

While working within the Harm Reduction Paradigm, the APN will coordinate the health outreach program - supervising staff and building linkages with community health service providers and public health agencies. The APN will provide health care, education and prevention services on the Street Medicine Team, Health Outreach Bus, CTA, and in other outreach settings.

Key Responsibilities:

  • Conduct physical assessments and offer referrals for follow-up care across various outreach settings (Street Medicine Team, CTA program, Health Outreach Bus, youth programs).
  • Build trusting relationships with clients.
  • Provide Medication Assisted Treatment for Opioids and support for Substance Use Disorders.
  • Complete accurate data entry in EHR and ETO databases; maintain documentation as required.
  • Supervise Nurse Practitioners.
  • Lead the design and maintenance of the health outreach program; recommend and implement expansions.
  • Establish and maintain relationships with health service providers.
  • Develop and ensure compliance with treatment and safety protocols.
  • Recruit, hire, and support health program staff, interns, fellows, and volunteers; coordinate their training and supervision.
  • Participate in and lead internal and external trainings and health services staff meetings.
  • Represent TNM in community institutions and participate in public relations and fundraising activities.
  • Travel to other outreach settings as needed.
  • Perform other duties as assigned.
  • Eligible for partial reimbursement of monthly cell phone bills.

Position Qualifications:

  • Must possess an M.S.N. degree and current Advanced Family or Adult Practice Nursing License.
  • Possess DEA license or willing to obtain one.
  • Minimum 5 years' nursing experience.
  • Must have scheduling flexibility for daytime, evenings, and weekend.
  • Demonstrated experience in supervision and counseling patients/clients.
  • Demonstrated experience with medically underserved populations.
  • Demonstrated experience in counseling patients/clients.
  • Excellent interpersonal skills.
  • Ability to react quickly in emergency situations; demonstrated "street smarts".
  • Physically able and willing to work in various settings, including outdoors; this position requires riding the Health Outreach Bus for Bus shifts and vans for Street Medicine and/or the CTA.
  • Familiarity with referral sources.
  • Current driver's license and good driving record; access to reliable personal transportation.
  • Ability to work collaboratively in a team environment committed to fulfilling the mission, supporting each other, and having fun along the way.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. The employee is occasionally required to stand, walk, sit, reach with hands and arms, climb or balance, and stoop, kneel, crouch, or crawl. The employee frequently is required to use hands or fingers, handle or feel objects, tools or controls. The employee is regularly required to talk or listen. The employee must occasionally drive an automobile and the outreach bus; and lift and/or move 20 pounds.

Benefits: Full-time staff are entitled to full benefits after 30 days of employment, including medical, dental, vision, 401k, and more, starting 30 days after their employment commences.
