The Night Ministry is a Chicago-based nonprofit that provides human connection, housing, and health care to individuals struggling with housing stability or poverty. Our work is centered around two core service areas: Outreach & Healthcare and Youth Housing. Through our Outreach & Health programs, we meet people where they are, on CTA platforms, in encampments, at libraries, and across city neighborhoods; offering consistent, compassionate care and vital health services.
Our Youth Housing programs provide shelter and supportive services to young adults, helping them build stability and work toward their goals in safe, affirming environments. What You'll Do
As the CTA Outreach Case Manager, you'll support our Street Medicine and CTA outreach efforts by working directly with clients who are often unhoused or medically vulnerable. Your focus will be on building relationships and connecting individuals to housing, health, and supportive services through case management, resource navigation, and ongoing follow-up.
Your work will include:

Providing case management to clients served by the CTA Outreach Program through on-site support and referrals.

Developing and maintaining strong relationships with partner agencies in housing, healthcare, mental health, and addiction services.

Accompanying Street Medicine staff to locations including CTA platforms, the Harold Washington Library, and The Night Ministry's Health Outreach Bus.

Using motivational interviewing and harm reduction methods to engage with clients in a non-judgmental, trauma-informed way.

Accurately tracking client engagement through systems like Electronic Health Records (EHR) and Efforts to Outcomes (ETO).

Driving the Outreach Van as needed and maintaining a strong, supportive presence during street outreach efforts.

Attending internal and external meetings, supporting public relations and fundraising efforts, and contributing to agency representation in the community.
What We're Looking For
We're looking for a committed, compassionate professional who brings strong interpersonal skills, deep cultural competence, and a belief in meeting people where they are.
Qualifications:

Bachelor's degree in social services or related field or equivalent experience.

At least 2 years of experience in outreach, health, or case management.

Trauma-informed, LGBTQ competent, and street-culture aware.

Strong organizational skills, clear professional boundaries, and the ability to work both independently and on a team.

Valid driver's license and access to a car; willing to drive for outreach efforts.

Comfortable working with diverse populations, including unhoused individuals, sex workers, LGBTQ communities, and youth.
Bonus if you:

Are bilingual in Spanish and English.
