Job Summary:

Responsibilities:
1. Collaborate with healthcare professionals to develop and implement individualized patient care plans.
2. Provide direct patient care
3. Collect patient health information, document observations, and report changes in patient conditions to the supervising nurse or physician.
4. Maintain accurate and up-to-date medical records, including electronic health records, ensuring confidentiality and compliance with legal and regulatory guidelines.
5. Ensure the cleanliness and organization of patient rooms, medical equipment, and work areas.
6. Collaborate with the multidisciplinary healthcare team to coordinate and facilitate patient care, ensuring continuity across shifts and departments.
7. Adhere to infection control and safety protocols, promoting a safe and healthy environment for both patients and staff.
8. Participate in ongoing professional development activities to maintain and enhance nursing skills and knowledge.
Qualifications:
1. Valid LPN/LVN license in the state of NY.
2. Prior experience as an LPN in a healthcare setting preferred.
3. Ability to prioritize and efficiently manage multiple tasks in a fast-paced environment.
4. Excellent interpersonal and communication skills, both verbal and written.
5. Strong critical thinking and problem-solving abilities.
6. Proficiency in using electronic health record systems and other healthcare software.
7. Knowledge of medical terminology and understanding of disease processes.
8. Ability to provide emotional support and work compassionately with patients of diverse backgrounds.
9. Flexibility and adaptability to changing healthcare needs and technologies.
10. Current CPR certification.
