What are the responsibilities and job description for the Admissions Coordinator position at The Oakmont Guest Care Center?
Summary of role:
Proficiency in communication about a new referral or readmission and dynamic conversation about the patients’ approval status with their insurance company. This role requires high communication in morning meetings about pending admissions, emails, to all department heads about an admission and organization of the admission. Additionally, Admissions Coordinator will confirm all insurance benefits coverage meets the standards of admissions dictated by policy. Admissions Coordinator will oversee bed assignments, room readiness, and completion of the preliminary paperwork and documents that are required for a new admission. Professionalism is of the utmost importance in this role, as AC will handle inquiry calls from hospitals, families, etc., and provide appointments that include tours and additional information.
Duties Include the following:
*Completing Admissions Paperwork and gathering all required documents for an admission
*Uploading any documents and Admissions paperwork to patient file in a timely manner
*90% of all admissions paperwork is ideal to be completed before admission (Admissions will not be delayed if unable but must be a priority to get this in timely)
*Verification of Benefits and Financial approval documented in the referral group email
*Responsible for all new admissions insurance authorizations and communication to the referral group in email once it has been sent, as well as, following up with insurance companies to ensure that they received the proper documentation.
*Communication with DBD about any pending admissions/dates/times and/or inquiries of leads that are interested in The Highlands Guest Care.
*Communication in morning meeting to update all Directors of any pending admissions/referrals/leads.
*Communication with patients’ families before the admission and after the admission to ensure all is going well, if there are further concerns you will refer them to SW, Department Head, and/or Administrator.
*Uploading the new referral’s clinicals in the admissions referral group in a timely manner using the template from the Regional Director of Business Development, clinicals will come in via Fax, and or NAVI, EPIC, Allscripts depending on the hospital.
*Dress is business casual unless it is a themed day by the Administrator at The Highlands Guest Care.
* Communication with the BOM for potential long-term residents/Medicaid pending.
*Verifying days with other SNF’s BOM’s if there are any other SNF Stays.
*Responsible for keeping the conversion rate about 75% along with the Director of Business Development
*Responsible for keeping up the with the approval process to ensure that it is moving quickly and smoothly

Pay: $25.00 - $28.00 per hour
