Position Title: Integrated Care Liaison
(Position Available Covering the Middle TN Region & Position Available Covering the East TN Region)

As the Integrated Care Liaison, you will serve as a bridge between healthcare providers, clients, and community resources to ensure effective mental health care. The Integrated Care Liaison will heavily be involved with building relationships with discharging provides, working closely with healthcare teams to ensure transitions of care are completed, and enrolling into programs to ensure quality care. This role focuses on fostering collaboration, improving care continuity, and supporting clients' mental health outcomes.

What you bring to this role:

- Bachelors degree in a behavioral health related field such as counseling, social work, or psychology.
- Minimum of 2 years of relevant work experience (e.g., experience in healthcare, preferably in customer / patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach)
- Valid Drivers License
- Proof of Automobile Insurance and Reliable Transportation
