AGENCY: The Open Door, Inc. (TOD) is a Pittsburgh-based nonprofit whose mission is to provide supportive housing, among other social services to people with HIV and chronic homelessness in order to improve health outcomes. TOD serves high-risk populations through housing first, harm reduction models. Our services include low-barrier housing programs, non-medical case management, peer-led service coordination, and a representative payee program.

JOB SUMMARY: The Case Manager will provide client-centered, non-medical case management activities to TOD participants with a focus on housing stability case management. The Case Manager will work closely with the Housing Navigator, Service Coordinator, Program Manager, and other staff to balance a shared caseload of about 50 program participants, primarily those living at our agency-owned, 14-unit residential building, as well as those enrolled in our rapid rehousing program, the SURE Housing Initiative.

The person occupying this role is expected to independently manage their assigned caseload while collaborating with other program staff, meet with assigned participants at minimum once per month, assist participants with goal setting and implementation, complete timely documentation and data collection, and overall support program participants in acquiring/maintaining safe, stable housing. Additionally, it is critical that the Case Manager maintain confidentiality and informed consent, as well as adhere to housing first, harm reduction principles in accordance with the mission of TOD.

CORE DUTIES AND RESPONSIBILITIES:

Intake, Assessment, and Documentation (20%)

  • Assist with gathering required participant information and documentation for intake, enrollment, Ryan White Certification, medical chart reviews, and other program requirements
  • Complete initial assessment of service needs, identify gaps in care, then provide guidance and assistance in accessing medical, social, community, legal, financial, and other needed services, including conducting appropriate referrals
  • Stay up-to-date on community resources and assist with maintaining an up-to-date community resource guide
  • Maintain accurate and up-to-date participant case files to ensure compliance with all agency, state, federal, and grant regulations
  • Complete documentation and data collection in a timely and accurate manner, including daily case notes and monthly reports

MINIMUM QUALIFICATIONS:

  • Bachelor’s Degree in human/social services field (social work, human services, psychology) or related field
  • Demonstrated computer proficiency and working knowledge of the internet
  • Great organizational skills, time management skills, and communication skills (written and oral)
  • Ability to work with diverse populations with respect, dignity, and sensitivity, including people with HIV, people in active substance use, people transitioning from chronic homelessness, and the LGBTQIA community
  • Willingness to learn about, and adhere to, the principles of harm reduction, housing first, cultural humility, and trauma-informed care, as well as best practices for working with people with HIV/AIDS

PREFERRED QUALIFICATIONS:

  • Bachelors of Social Work (BSW); knowledge of generalist social work practice
  • Personal or professional knowledge of harm reduction, housing first, and/or trauma-informed care
  • Familiarity with activity-tracking (note-taking) software
  • Previous experience working with housing/homelessness, HIV/AIDS care, people who use drugs (PWUD), or other social services
  • Familiarity with health and human service resources in the service area (Greater Pittsburgh)
