Position Summary
The Nurse Navigator provides support to clients undergoing treatment for alcohol and substance use disorders and mental health crises. This position will coordinate care, provide education, and assist clients through their recovery journey in an outpatient setting.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
· Perform nursing assessments and triage of client care needs for new and ongoing clients.
· Work closely with the Treatment Team, Medical Director, and Crisis Intervention Specialists to ensure comprehensive care delivery.
· Administer Sublocade and Brixadi injections as necessary.
· Provide and administer Narcan to those in need.
· Process Suboxone prescriptions as needed for clients.
· Educate clients on managing health conditions, proper administration of Narcan, and self-care strategies.
· Educate staff on proper administration of Narcan.
· Maintain accurate and up to date client records in accordance with regulations and facility policies.
· Promote client advocacy and ensure clients’ needs are met with respect, confidentiality, and empathy.
· Support clients in connecting with support groups, educational resources, and community services to further their recovery.
· Perform other duties as assigned
Minimum Qualifications
· Current licensure in this Commonwealth as an RN or LPN and a degree from an accredited school of nursing with at least 1 year of experience in a health or human services agency, preferably in a drug and alcohol setting.
· At least two (2) years of experience in An outpatient care or community-based health service preferred.
Knowledge, Skills, and Abilities)
· Knowledge and understanding of nursing processes.
· Knowledge and understanding of the disease of chemical dependency.
· Knowledge of the family disease of chemical dependency.
· Knowledge of affective, behavioral and cognitive consequences of alcohol and other drug dependencies.
· Knowledge of alcohol and other drug rehabilitation techniques.
· Ability to work with alcohol and other drug rehabilitation techniques.
· Ability to work with dually diagnosed clients.
· Ability to work with clients and gain their confidence and cooperation.
· Ability to comply with all agency, SCA and BDHP paperwork.
Job Type: Full-time
Pay: $38,000.00 - $45,000.00 per year
