The Position Summary
Provides education to professionals and community members about occupational therapy, including optimization of physical and cognitive abilities, home adaptations, and engagement. Works with staff and members to plan, research, and facilitate the Supported Aging Program – focusing on people with I/DD over 50 and/or have a diagnosis or are exhibiting symptoms of Alzheimer’s/Dementia. The OT/Program Manager will focus on assessments, interventions, program upgrades, and supplies necessary to provide the proper interventions and support to assist Members as they age in place.

The Position Summary

The OT/Program Manager will focus on assessments, interventions, program upgrades, and supplies necessary to provide the proper interventions and support to assist Members as they age in place.
Job Functions
- Provides skilled occupational therapy services to persons with I/DD living with dementia in any of The Opportunity Tree Programs.
- Evaluates patients with cognitive and ADL physical functioning challenges and develops appropriate plan of care to optimize cognitive and physical functioning, following regulatory and clinical practice standards.
- Utilizes standardized assessment and evidence-based practice to support clinical interventions, including but not limited to therapeutic activities and exercises, use of supportive and assistive devices, and cognitive interventions.
- Develops, maintains, and enhances a specialized knowledge base and professional skills regarding occupational therapy for persons living with dementia.
- Continually seeks opportunities to enhance knowledge of excellent dementia care.
- Completes courses when requested focused on OT engagement of persons with IDD living with dementia.
- Must be up to date on research and best practices for occupational therapy for persons with IDD living with dementia.
- Regularly seeks feedback from HOV teams, staff, and support staff and applies it to improving performance.
Requirements
- Minimum of 18 years of age.
- Current driver license with Non-Restricted Motor Vehicle Record and compliance per the requirements of The Opportunity Tree’s insurance carrier.
- Communicate in English; speech, write, and read.
- Excellent verbal and written skills.
- Experience: Two (2) years in the Intellectual and Developmental Disabilities (IDD) field in a vocational, rehabilitation, or similar setting preferred.
- Education: Must be a graduate of an accredited program in Occupational Therapy (OT) and must obtain an OT License within six (6) months of hire.
- Knowledge: Intermediate in MS Outlook, MS Word, MS Excel, web browsing.
