Description
Provides personal care services to clients in their residence as directed by the Care Coordinator.
Essential Duties and Responsibilities:
Teamwork with the following and all other duties and responsibilities assigned.
- Provides primary care including all ADL’s, including but not limited to bathing, dressing, toileting, restorative nursing, and vital signs.
- Monitors the safety of the client.
- Provides restorative care for clients in their residence as per individual needs, physician order and support plan.
- Attends meetings with other staff to discuss client needs and to participate in utilization review, case management, and quality assessment activities and analysis. Attends other meetings as required or appropriate for position
- Maintains on going communication with the Care Coordinator regarding client needs.
Other Responsibilities:
- Demonstrates respect and regard for the dignity of all clients, families, and fellow employees to ensure a professional, responsible, and courteous environment.
- Initiates immediate intervention in abusive situations to protect the client and report allegations according to facility policy.
- Notifies Care Coordinator and families when there are changes in condition and emergencies.

Education and/or Experience
High School diploma or equivalent; Complete annual state mandated training requirements (Regular In-services as well as any external training).

While performing the duties of this job, the employee is regularly required to talk or hear when communicating with clients, family members, staff, and outside business contacts. The employee is frequently required to stand while conversing with various individuals; walk throughout the unit; sit at a desk; use hands to finger, handle, or feel when using the telephone or computer, and attending to the physical needs of the client; and reach with hands and arms when obtaining supplies, and attending to client needs. The employee is occasionally required to climb stairs or balance when assisting others with clients; stoop or crouch to communicate with clients and to place items in or get items from low drawers or shelves; pump a foot pedal to raise or lower a client’s bed; and use his/her sense of smell to detect odors within the home and emanating from the client. The employee must occasionally lift or move up to 100 pounds when moving clients by wheelchair. The employee must occasionally lift up to 10 pounds, which is generally supplies such as reams of paper, files, and forms. Specific vision abilities required by this job include: close vision and peripheral vision in order to monitor client behavior at close range; close vision for computer and paper detail work; color vision to assess changes in coloring of clients' skin or wounds, and to review color-coded spreadsheets or documents; and depth perception and ability to adjust focus from far clients to near clients and from computer to desk or unit hallways.
