The Social Services Director is responsible to ensure the medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with policies and procedures and current Federal and State guidelines and regulations. This position serves as the licensed Social Worker for the Health Center.

Principle Duties:
- Develops a comprehensive social history and assessment of the resident that includes problems and strengths, special needs and preferences (social, religious and/or cultural) and the implications for the care plan.
- Assists the resident/family with the transition to the new environment, orients the resident/family to the facility, services, limitations and residents rights.
- Acts as a mediator to resolve resident and/or family concerns/conflicts that may arise regarding care and treatment, admission and discharge procedures, physician care and treatment and any other situations that could negatively affect the resident.
- Participates in the development of the individuals care plan including the goals and approaches for the Social Services component.
- Actively participates in the coordination of resident care services and evaluates the ongoing needs of the residents through attending resident care plan meetings, rehabilitation and resident/family meetings, departmental staff and annual policy review meetings and monthly quality assurance meetings.
- Ensures that any Advanced Directives are complete and maintained and placed in resident charts.
- Coordinates the functions of the Health Center Behavior Management committee, policies and documentation.
- Assists with the coordination of Health Center admissions and assessments.
- Coordinates discharges/transfers to ensure the appropriateness and continuity of care. Develops a comprehensive discharge plan in coordination with residents/families and multidisciplinary staff.
- Establishes and maintains positive relationships with residents, family members, physicians, community leaders, state and federal surveyors, hospital case managers and other professionals.
- Provide/identify referral, agency and/or community resources to meet the needs of the resident/family.
- Coordinate Support Groups, participates in policy decisions (as they affect resident life issues) and advocates for Residents Rights.
- Completes all mandatory documentation in a timely (determined) fashion as established by applicable regulations.
- Maintains confidentiality of all admission information and resident records following HIPPA and the communitys Code of Ethics.
- Other duties as assigned by supervisor.
What you will bring:
- Minimum of 3 years experience in hospital or skilled nursing setting required. Prefer at least 5 years experience.
- Excellent oral, written and interpersonal skills. Must present a professional manner and be able to deal with situations that may be stressful.
- Must have experience working with Title 22 and OBRA Regulations, including, but not limited to, Medicare, PPS, MDS, Care Planning and HIPPA.
- Ability to multi-task and prioritize effectively, and work closely with residents, families and staff.
- Bachelor degree in Social Work, Gerontology or related field. Masters degree preferred.
