Position Overview: The Assisted Living Care Coordinator is responsible for providing routine daily care to residents, including personal care, homemaking, escort, and other supportive services. As a representative of the Assisted Living Program and our organization, it is essential that the Care Coordinator performs their duties professionally, with a positive attitude, and in alignment with our mission, vision, resident Bill of Rights, policies, and procedures.
Key Responsibilities:
- Provide routine daily care to residents in areas such as personal care, homemaking, and escort services.
- Assist residents with activities of daily living, ensuring their comfort and safety.
- Maintain a clean and organized living environment for residents.
- Escort residents to appointments, activities, and social events as needed.
- Foster a positive and supportive environment that promotes resident wellbeing and satisfaction.
- Act in accordance with the resident Bill of Rights and organizational policies and procedures.
- Communicate effectively with residents, families, and other staff members to ensure continuity of care.
- Monitor and report any changes in residents’ health or behavior to the appropriate personnel.
- Participate in ongoing training and professional development opportunities.
Qualifications:
- Must have Active NYS HHA Certification
- Experience in a caregiving role within an assisted living or similar healthcare setting is preferred.
- Strong interpersonal and communication skills.
- Compassionate and patient-focused approach to resident care.
- Ability to perform tasks related to personal care and homemaking.
- Positive attitude and professional demeanor.
Per Diem Opportunities
Salary: $18.59 per hour
