Position Description : The Outlook at Windhaven is currently looking for a Speech Language Pathologist to join our fantastic team!

ESSENTIAL DUTIES :
Model a service attitude towards others, is friendly and courteous to residents, patients, visitors, and co-workers
Take ownership in solving problems and take the initiative to make things better
Ensure resident, patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
Maintain good attendance record
Document all assessments, reassessments, progress notes, discharge summaries and other clinical work in the medical record per departmental policy and procedure
Provide resident, family and staff education pertaining to Speech Therapy modalities to ensure understanding and proficiency of care
Demonstrate good clinical judgment in all aspects of resident treatments in which trained to provide
Orient, instruct, and direct work activities of Clinical Fellows and Speech Therapy students.
Supervise and review documentation written by the Clinical Fellow as per state practice act
Screen residents to determine their potential benefit from Speech Therapy intervention
Develop, direct and / or participate in Speech Therapy programming for the residents
Attend in-services and other meetings as required on an ongoing basis
Maintain continuing education per state licensure requirements
Other duties as assigned by the Director of Rehab
MINIMUM QUALIFICATIONS :
Licensed Speech-Language Pathologist in the current state
Masters degree in Speech Language Pathology with Clinical Certificate of Competency or in Clinical Fellowship Year
Proficiency with Nethealth documentation system preferred
Knowledge of therapy industry and federal, state, and local government regulations
Demonstrated ability to organize, prioritize and complete multiple tasks in a dynamic environment
Demonstrated efficiency strategies for fast paced environment
Experience in SNF / LTC / IL / AL / OP setting or mature population rehabilitation and wellness needs
Experience with PDPM treatment and reimbursement policy
Basic computer skills, Outlook, Word, Excel
Excellent customer service skills
Genuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients
Ability to perform work tasks within the Physical Demand Requirements
Ability to perform Essential Duties
Ability to comply with the Resident Bill of Rights and the Employee Responsibilities
Ability to comply with organizational and departmental safety policies and procedures
