- Assesses patient’s health problems and needs and develops plan of care.
- Provides direct skilled nursing care as per physician orders.
- Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.
- Document medical records and prepare other reports as needed.
- Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
- Completes home health aide supervisions and orientations as needed.
- Co-signs orders and provides oversight of care for LPNs.
- Graduate of an accredited school of nursing – required.
- Current license to practice as a registered nurse in the state of Massachusetts – required.
- Current CPR – required.
- One year nursing experience, preferably med-surg - preferred
- Possess a valid State of Residence Driver’s license and reliable transportation – required.
Salary : $76,976 - $122,200
