Position Summary:
The Social Services Assistant supports residents and families in addressing personal, emotional, and social needs within a long-term care setting. This role helps ensure residents maintain their highest level of well-being and dignity, facilitating adjustment to facility life, and advocating for resident rights.
Key Responsibilities:
Assist in the assessment and development of individualized care plans for residents.
Support residents during admission, discharge, and transitions within the facility.
Provide emotional support and crisis intervention to residents and families as needed.
Maintain accurate and timely documentation in compliance with federal, state, and facility standards.
Help coordinate services such as counseling, transportation, legal aid, and guardianship.
Promote resident autonomy and advocate for their rights and preferences.
Participate in interdisciplinary team meetings and care plan conferences.
Educate residents and families about facility policies, care options, and community resources.
Monitor and respond to resident grievances and concerns.
Assist with advance care planning and end-of-life support.
Assist with referrals for admissions, give tours to families and potential new admissions, assist with daily census
Qualifications:
High school diploma or GED required; associate’s or bachelor’s degree in social work, psychology, or related field preferred.
Experience in long-term care or healthcare setting preferred.
Strong interpersonal and communication skills.
Knowledge of state and federal regulations related to resident care and rights.
Ability to handle sensitive and confidential information with compassion and professionalism.
Basic computer skills and experience with electronic health records (EHR) preferred.
