Job DescriptionResponsible for establishing and maintaining positive relationships with patients and referral sources, responding to patient requests and concerns. Essential Job Functions / Responsibilities related to community relations; Works with the Director of Business Development and Clinical Team to establish community education for clinical programs.Establishes and maintains positive working relationships with current and potential referral and payer sources.Builds and monitors community, customer, and payer and patient perceptions of Hospice of Missoula as a high quality provider of services.Maintains comprehensive working knowledge of Hospice of Missoula and Hospice markets including government agencies, major payer groups, key referral sources, and competitor's market positioning.Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Hospice of MissoulaEssential Job Functions / Responsibilities related to Transitional care coordination; Evaluate the appropriateness of patients referred for home health or hospice services Confirm the level of care most appropriate for the patientConduct an on-site assessment to facilitate safe and effective transition from in patient to home health or hospiceEngage patients, family, caregivers as well as the inpatient care team to obtain appropriate information to support discharge to home health or hospice careHolding continuity of care as a priority, the TCC communicates and coordinates with the inpatient care team supporting the patient including hospital case management and nursing, patient's physicians and other healthcare providers involved in the patient's care at home. The TCC will attend conferences as requested, facilitate orders, evaluate patient's care (medical, pyscho-social and practical) needs, including medical eligibility of services, anticipated needs for equipment, medication and transportationResponsible for BPCI / ACO Program growth in collaboration with Administrator, Director of Business Development and Director(s) of NursingResponsible for referral source satisfaction with the BPCI / ACO ProgramServes as a clinical resource for patients, families, physicians, and facility staff for appropriate home placement, medication reconciliation, goals of care and identification of necessary support at home, as well as problem solving barriers or challenges related to the patient returning homeThe above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job-related tasks other than those stated in this description.Position Qualifications Minimum of 3 years of nursing experience required (MA,LPN,RN)Minimum of 3 years or related job experience in the business and marketing field.Must have direct patient care experience within the last five yearsPreferred but not required - One year experience in home health and / or hospice setting preferred within the last three yearsRecent experience in acute-care hospital setting; knowledge of discharge planning / care management strongly preferred.Ability to market aggressively and deal tactfully with customers and the community.Demonstrates good communications skills, negotiation skills, and public relations skills.KNOWELEDGEAbility to utilize reports and trends to support efforts and to understand clinical status and progress as it pertains to patient careMust have clinical knowledge and critical thinking ability to create a viable and effective patient care plan and identify barriers in services, effectively conduct verbal and / or written report of patient care needsUnderstanding home health and hospice services regulations, Title XXII, Medicare Conditions of Participation, reimbursement systems, home health and / or hospice accreditation requirementsKnowledgeable about disease processes and prognosis and how illness progresses clinically and functionallyKnowledgeable of how the range of home care services can help patients with a wide range of needs from post-surgical care to palliative care, to end of life care.SKILLSSelf-starter with a high degree of initiative, motivation, flexibility, energy and creativityAbility to work as part of a team as well as form harmonious working relationships with internal and external customersExcellent collaborative and problem-solving skills with customers and managementExcellent organizational skillsMust have excellent telephone, interpersonal and written communication skillsStrong commitment to teamwork and customer service, both internally and externallyExperience with Microsoft Office suite and electronic health record strongly desiredMust be flexible with scheduleMay be asked to work in field providing home care services as per changes in business needs and as skills and training allowMust be able to develop an organized work plan in a high-volume environment with rapidly changing prioritiesTake action with individual patients to help reduce hospital readmissions and decrease ED utilizationMaintains high risk for readmission patients on active caseload, making phone calls to discharge patients until it is determined that the patient no longer requires this level of interventionConnects patient to appropriate resources to maintain their health at home including, but not limited to : arranging outpatient appointments, assuring appointment transportation needs are met, confirm prescriptions can and have been filled, inquire about any barriers to health, etc.Attends patient / family / staff conferences at hospital / SNF to assist with identification of patient needsFacilitates timely initiation of care to home health or hospiceUtilizes reports and trends to support efforts and to understand clinical status and progress as it pertains to patient care.The employer for this position is stated in the job posting.
