Job Overview
We seek a highly motivated Clinical Liaison to drive referrals and admissions for our large skilled nursing facility that is currently undergoing a transformational renovation. Help us connect patients, families, and healthcare providers with exceptional post-acute care and rehabilitation services in the Texas City area and beyond, from League City and Clear Lake down to Galveston. Key Responsibilities:
- Build and maintain strong relationships with hospitals, physicians, case managers, discharge planners, and other referral sources.
- Evaluate and screen potential patients to ensure eligibility for admission based on medical needs and insurance coverage.
- Effectively communicate the facility’s services and exciting renovations to referral sources and families.
- Provide education and guidance to patients and families on post-acute care options.
- Plan and host events, tours, and community outreach efforts to engage potential residents and families.
- Collaborate with facility leadership to develop and implement strategic growth initiatives.
- Track referral trends and identify opportunities to increase census.
Qualifications:
- Licensed nurse (RN, LPN/LVN) or therapist (PT, OT, SLP) preferred, with experience in skilled nursing or rehabilitation.
- Clinical expertise of medical conditions, treatment options, and disease processes preferred.
- Strong relationship-building and excellent communication skills to effectively interact with patients, families, and healthcare providers.
- Knowledge of insurance, Medicare/Medicaid, and admission criteria.
- Self-driven, goal-oriented, and excited to represent a newly renovated facility.
