You will work under the supervision of a registered nurse and be responsible for observing patients, reporting observations, and documenting care provided. This is a time-sensitive role that requires compassion, empathy, and a commitment to delivering high-quality care.
Job Summary :
- Perform personal care needs and attend to patient requests promptly
- Ensure patient safety and use equipment properly
- Provide personal care including baths, oral hygiene, and changing bed linen
- Assist patients with dressing and undressing
- Plan and prepare nutritious meals
- Assist with feeding patients
- Take and record vital signs
- Provide proper care and observation of patient's skin
- Report patient's condition and significant changes to the Case Manager
- Assist with ambulation and exercise
- Offer and assist with personal hygiene needs
- Perform simple procedures as ordered
- Keep patient's living area clean and orderly
- Adhere to documentation and care procedures
- Participate in quality assessment performance improvement activities
- Respect patient and family / caregiver environment and personal needs
